Hypersensitivity pneumonitis has many clinical, roentgenographic and pathophysiologic features that may be indistinguishable from idiopathic pulmonary fibrosis. But, there are characteristics of hypersensitivity pneumonitis that may be enable a specific diagnosis. Because elimination of the etiologic agent may prevent continued damage and corticosteroid therapy may at times produce dramatic improvement, all patients with idiopathic pulmonary fibrosis should be evaluated for hypersensitivity pneumonitis. We reviewed clinical findings of 49-year-old a male farmer patient who was suffered from coughing and dyspnea. Hypersensitivity pneumonitis was confirmed by skin testing using diluted antigen, and transbronchial lung biopsy. After removing the patient from the environment and corticosteroid therapy starter, symptoms were much improved.
